We take care of children of all ages, adults and older adults.

A referral (order) from a provider including an ICD-10 diagnosis code is required.

Services include:

• Individualized Nutrition Counseling
• Diabetes Self-Management Education and Training (DSME/T)
• Breastfeeding and Lactation Support

Appointments are available in-person and/or as video visits

We accept insurance:

• Commercial, Medicare, Managed Medicaid and fee-for-service
• Insurance coverage varies by plan and diagnosis
