What to bring to your appointment
- All insurance cards
- Valid form of identification
- Referral from Clinical Center of Excellence
Referral requirements
- Must be directly from the Clinical Center of Excellence in which you are registered
- Must have primary diagnosis listed
- Must have a referral effective date as well as a termination date listed
Patient Responsibilities
- Patients must provide a referral from their Clinical Center of Excellence (CCE) prior to their scheduled appointment. If you do not have a referral, please contact your case worker or care coordinator at your CCE location.
- Patients are responsible for informing their Clinical Center of Excellence (CCE) care coordinator/case worker of all upcoming appointments scheduled outside of your CCE location
- Inform scheduler/registrar if you are a survivor, responder, part of FDNY, or reside outside of New York state
- Provide the name of your CCE case worker/care coordinator as well as contact information and at which location you are registered to
