What to bring to your appointment

  • All insurance cards
  • Valid form of identification
  • Referral from Clinical Center of Excellence

Referral requirements

  • Must be directly from the Clinical Center of Excellence in which you are registered
  • Must have primary diagnosis listed
  • Must have a referral effective date as well as a termination date listed

Patient Responsibilities

  1. Patients must provide a referral from their Clinical Center of Excellence (CCE) prior to their scheduled appointment. If you do not have a referral, please contact your case worker or care coordinator at your CCE location.
  2. Patients are responsible for informing their Clinical Center of Excellence (CCE) care coordinator/case worker of all upcoming appointments scheduled outside of your CCE location
  3. Inform scheduler/registrar if you are a survivor, responder, part of FDNY, or reside outside of New York state
  4. Provide the name of your CCE case worker/care coordinator as well as contact information and at which location you are registered to

No, NewYork-Presbyterian is not a Clinical Center of Excellence for the World Trade Center program.

No, you can only schedule services that are directly related to your diagnosed World Trade Center illness.

The patient centered appointment service team is only equipped to handle billing and insurance inquiries related to WTC visits.
