What is a Patient-Centered Medical Home?

A patient-centered medical home (PCMH) facilitates partnerships between patients and their physicians. Your Primary Care Provider heads your PCMH team and includes all your healthcare providers-from physician specialists to nurses and technicians. The PCMH model puts you at the center of a team whose members all share a common goal: keeping you healthy!

 

Your PCMH Healthcare Team

  • Provide you with the care you need when you need it. We offer convenient same-day and next-day appointments, after-hours phone access, and extended hours, such as early mornings and evenings.
  • Customize your care to meet your needs and expectations.
  • Treat you with the respect you deserve as a full partner in your healthcare.
  • Help you set appropriate health goals and work with you to meet them.
  • Ensure that you understand what you need to do successfully to meet your goals and answer any questions you may have.
  • Offer evidence-based care to meet your goals and needs.
  • Suggest supplemental resources for patient education and self-management support, such as classes or support groups that help you accomplish your goals. A digital/paper copy of resources can be given after visit or through the portal.

Your Primary Care Doctor

As the leader of your healthcare team, your primary care provider (PCP) has additional roles to play. Your PCP will:

  • Take time to learn about yourself – your lifestyle, family situation, family history, and preferences.
  • Provide preventative and wellness care, short-term care for acute illnesses, and long-term care for chronic diseases and conditions.
  • Serve as a gateway to members of the medical community who are on your healthcare team and coordinate your care with them.
  • Notify you of test results promptly.
  • Remind you of your responsibilities as a patient.

And don’t forget the most important person on the team – you! As the central focus and most important member of your team, you are a full partner when it comes to your healthcare. It is your responsibility to supply your team members with the information they need to provide the best possible care.
