Yes, you can receive audiology services at other locations and still enroll in NewYork-Presbyterian Hospital’s Community Hearing Health Collaborative Program. How can I refer my child to the Community Hearing Health Collaborative program? For referrals, ask your doctor to complete the CHHC referral form or to contact us via [email protected] or 212-305-3340. Can the community health workers accompany patients to appointments? Our bilingual community health workers maintain a relationship with the families to:Engage them with medical educational and social servicesVisit families at their homes to provide one-on-one supportAdvocate and accompany patients to appointments as needed Is this service free? Yes. There is no cost to services provided by Community Hearing Health Collaborative. How old does my child have to be to qualify for the program? The program is for children 0-3 years old who fail infant hearing screening or have received a hearing loss diagnosis.
