A Patient-Centered Medical Home
The NewYork-Presbyterian Queens Jackson Heights Family Health Center is designated as a Patient-Centered Medical Home, a facility that can integrate both primary care and specialty care through a treatment team personalized to fit a patient’s health goals. Your own primary care physician assembles those professionals needed on your care team. They will coordinate your care team across all settings, working in partnership with you and your family.

At the Jackson Heights Family Health Center, our team of experienced, compassionate includes:
- Adult internists and specialists from NewYork-Presbyterian Queens to address your comprehensive health needs
- Pediatric specialists and subspecialists (including Nutrition, Pulmonology) from NewYork-Presbyterian Queens and from NewYork-Presbyterian Komansky Children’s Hospital in Manhattan, with whom our physicians collaborate if your child needs to be referred for follow-up care
- Care coordinators to help you navigate the healthcare system and set up appointments
- Social workers who provide additional support
