While you visit the NewYork-Presbyterian Westchester (formerly Lawrence Hospital) website, you may have a few questions. In an effort to assist you, we have provided some answers to some of the more common questions our visitors may have. Where can I pay my bill?
You can mail your payment to NewYork-Presbyterian Westchester (formerly Lawrence Hospital), Billing Department, 55 Palmer Avenue, Bronxville, NY 10708. Or, you can pay your bill in person at the Cashier’s Department in the hospital’s main lobby.
How do I get a copy of my birth certificate?
Birth Certificates are issued by the Village of Bronxville. Contact the office at 914-337-6500 or write to Village Hall, Bronxville, NY 10708.
What is your main telephone number?
The main telephone number at NewYork-Presbyterian Westchester is 914-787-1000. If you need a physician referral, call 914-787-5000.
How can I apply for a job?

What health insurance plans do you accept?
Lawrence accepts a number of health insurance plans. Further, some doctors may accept different plans than our medical facility. To be sure, you should contact these physicians and ask which plans they accept.
How do I obtain a copy of my Medical Records?
In order to receive a copy of your medical records, a written authorization to release the records must be submitted to the Medical Records Department. There is a fee of up to $0.75 per page in most cases. If the records will be used for continuing medical care, the fee is waived when the records are sent directly to the physician/healthcare organization by the Medical Records Department. The Medical Records Department will contact the attending physician prior to releasing medical records to the patient. Medical records are released as soon as possible but may take up to 10 business days, as stated in the New York State Public Health Law. When copies of the medical records are available, the Medical Records Department will notify you and payment arrangements will be made. You may authorize another person to pick up your medical records. This must be in writing in your authorization and your designee must bring a government issued photo ID when picking up the records.
If you are requesting medical records other than your own, additional documents may be needed. Please contact the Medical Records Department at 914-787-3295 for further information. Completed authorization forms may be faxed to the Medical Records Department at 914-787-3010, mailed to NewYork-Presbyterian Westchester, Medical Records Department, 55 Palmer Avenue, Bronxville, NY 10708, or dropped off to the Medical Records Department located in Palmer Hall, 3rd floor.
How can I get copies of my Pathology Reports and Slides?
You must call 914-787-3265 or come to the Department of Pathology (2nd Floor) with at least a 24 to 36 hour notice to request copies of your report and slides. However, you will need to present a picture I.D. and complete a Pathology Release Form before it can be given to you. If you are asking someone else to pick-up slides and a report on your behalf, you must send in a signed letter giving permission for that person to do so. That individual will need to provide us with his/her own picture I.D. in order to get them released. (We reserve the right to call the patient to verify his/her authorization).
To pick-up slides, there is a preparation fee of $48.00 (one time fee). If you request that we mail the slides to a doctor’s office or another facility, there will be a fee of $60.00 for Overnight Delivery (Check to be made out to Lawrence Hospital. In the Memo write Pathology Slides).
If you want only a copy of your report, there is no cost to you for this, but authorization requirements are the same as above.
How can I get copies of my x-rays or other radiological films?
You should call 914-787-2110, with at least a 24 hour notice to request copies of your x-rays or other radiological images. These images are saved on a Compact Disc (CD). The cost of the CD is $10. However, you will need to present a picture ID and complete a Medical Release Form before it can be given to you. If you are asking someone else to pick up the CD on your behalf, you must send in a signed letter giving permission for that person to pick up your images. That individual will need to provide his/her own picture ID in order to get the CD. We reserve the right to call the patient to verify his/her authorization.
In the event, you request a copy of an image on film, there will be a fee of $20.00 for the first three pages of film and $5.00 per film thereafter up to a maximum of $50.00. The authorization requirements are the same as above. Payment can be made with cash or check and must be paid at the Cashier’s Office on the first floor of the hospital.
If you want a copy of your mammogram, we only provide those images on film. There is no cost for these copies. The authorization requirements are the same as above.
