Care Coordination
Our emphasis is on coordinated patient care. Within each clinical area, your medical team is supported by highly skilled nurses and patient navigators who guide you and your family members through each step of diagnosis, treatment and recovery.
Clinical Program Coordinators
Clinical Program Coordinators are experienced nurses who act as your point of contact while at the Center. They serve as a resource to patients by facilitating communications with your care team, family members and referring physicians.
They also help in answering questions you may have about your treatment; assist in scheduling diagnostic testing and appointments with specialists; follow up on recommended treatment plans and keep family members updated on your day-to-day progress.
Patient Navigators
Patient Navigators attend to all administrative details that keep the Center running smoothly, fielding patient calls from our Call Center, streamlining care for patients traveling from outside New York City, developing patient educational materials, assisting enrollment of patients in clinical trials, and following up with discharged patients.
