Scheduling Information
Hours of Operation
The hours for scheduling a routine examination are:
Monday - Friday
8 a.m. to 6 p.m.
For emergency examinations after hours or on weekends, please call your physician or visit an emergency room.
To Schedule an Appointment
Physician offices or patients with a written prescription from their physician may call one of the numbers below. Please note, we will need the following information to schedule an examination:
- Examination being ordered
- Patient's name, date of birth, phone number, and medical record number
- Patient's insurance information (policy number, phone number, state, pre-certification)
- Referring physician name and phone number
- Diagnosis or reason for exam
- ICD9 and/or CPT codes (You can find this on the order from your doctor.)
