We value your loyalty as our member. Due to a recent agreement, your membership in NewYork-Presbyterian
Community Health Plan will be transferred to Neighborhood Health Providers (NHP). This change will be in effect on
April 1, 2009. Neighborhood Health Providers is an established and respected managed care plan in New York City.
We share many of the same providers. If your doctor participates with NHP, you will be able to keep seeing your
doctor. For more details on the process please call us today at 800-261-4649. We thank you for your membership and will make sure that this transition goes smoothly.
