Geriatric Care Management Program

The Geriatric Care Management Program is a fee for service programs intended to help clients and their families achieve and maintain the greatest possible independence and quality of life in the community.Ê It is a personal and comprehensive approach to elder care that includes:

• Consultation with clients and families
• Home assessment of needs and discussion of options
• Development and implementation of home care plan
• Short and long term counseling
• Liaison between clients and family members living out of town
• Help with insurance claims
• Applications for Medicaid and other entitlements
• Referral to physician and legal counsel
• Evaluation and assistance with nursing home placement
