The Caregivers Service is a FREE service provided by HealthOutreach at New York-Presbyterian Hospital. Coordinated
by a certified social worker, it is designed to help caregivers identify and cope with the many complex issues
confronting them.
The Caregivers Service is a source of help and support to individuals and families involved in the care of any one
age 60 and over. As a participant, you can benefit from individual and family counseling, planning assistance, day
and evening support groups and informative lectures.
The Caregivers Service Coordinator, Dionisio Cruz, LMSW, is available Monday through Friday, 9 a.m. to 5 p.m. at
