The Long Island City Community Practice is a multidisciplinary primary health care center located at 36-11 21st Street, Long Island City, New York 11106. The comprehensive services available include health screenings, physical examinations and acute and chronic health care management. The full-time social worker provides services such as referral and liaison, health education and information, mental health assessment, counseling and support groups.

The social worker is available Monday through Friday from 9 am to 5 pm.
