Ambulatory Care Network Community Health Education and Outreach

School-Based Health Centers (SBHCs)

In 1986 the local New York City school district joined with CCHE to develop and implement School-Based Health Centers (SBHCs) located at two intermediate schools in Washington Heights. Since then, our services have expanded to include a total of seven SBHC sites serving fifteen middle and high schools with over 12,000 students in upper Manhattan.

Comprehensive, interdisciplinary clinical services are available at each School-Based Health Center including:

Medical and health services include comprehensive physicals, first aid, immunizations, management of acute and chronic illnesses, including asthma and diabetes, trauma response, laboratory tests, and referrals for specialty services.

Confidential reproductive health services include individual and small group education and counseling, pregnancy testing, emergency contraception, provision of effective contraceptives, case management for adherence to contraception, condoms, and sexually transmitted infection prevention education, testing and treatment.

Mental Health services include group and individual counseling, family counseling, case management, Interface with school personnel, crisis intervention, referral to entitlement services, legal assistance, day care, and other needed services, referral to other mental health resources, and stress management.

Health Education services include prevention information in the classroom, small group education, sports and recreational opportunities and individual health education and case management.
