Ambulatory Care Network Community Programs
WIN for Asthma
Childhood asthma is a major public health problem with rates highest among minorities, families with low-education attainment, and among those who reside in low-income communities. In Northern Manhattan, childhood asthma rates are three times the national average. Despite proximity to a large academic medical center, gaps in culturally appropriate asthma care and related supports lead to significant health risks for local children with asthma and compromised quality of life for their families.
To date, program staff have enrolled nearly 900 families of children with poorly controlled asthma, the majority of whom received a comprehensive home environmental assessment, trigger reducing tips, asthma information, and on-going support. Among all program graduates to date, hospitalizations decreased by 70%, emergency department visits decreased by 55%, and nearly 100% of caregivers reported feeling able to manage their child's asthma.
