WIN BACK
Coalition Mission Statement
In April 2012, NYP received a grant from the New York State Department of Health to serve as one of eight regional asthma coalitions throughout the state. The Washington Heights-Inwood Network Best Asthma Care for Kids (WIN-BACK) coalition was established as an expansion of NYP's WIN for Asthma program, leveraging existing partnerships and increasing efforts to support children with poorly controlled asthma and their families.
In Year 1, WIN-BACK will focus efforts on patients with two or more asthma-related ED visits during the previous 12 months. Beginning in November 2012, a nurse practitioner will meet with these patients and their families utilizing a one-of-a-kind computerized curriculum for asthma teaching. The nurse practitioner will ensure that children are linked to an appropriate medical home and return for follow up appointments. These families will also be offered on-site support from a community health worker (CHW) and the opportunity to enroll in the WIN for Asthma care coordination program.
WIN-BACK will also focus efforts on improving coordination in pediatric asthma care throughout the hospital and the larger community. The WIN-BACK coalition will leverage existing partnerships with community organizations, schools, and asthma programs and initiatives in northern Manhattan, and identify new partners to enhance the coalition's aims. These partners will help to inform the coalition's work in the community and participate in coordinated efforts to improve pediatric asthma care and address health and social service needs for families throughout northern Manhattan.
