Coalition Mission Statement

In April 2012, New York Presbyterian received a five year grant from the New York State Department of Health to serve as one of New York State's eight regional asthma coalitions. The Washington Heights-Inwood Network Best Asthma Care for Kids (WIN-BACK) coalition was established as an expansion of existing efforts by the WIN for Asthma program to support children with poorly controlled asthma and their families and to address the disproportionate burden of asthma in Northern Manhattan.

Beginning in mid-November 2012 WIN-BACK has implemented three main interventions in the Alexandra & Steven Cohen Children's Emergency Department at NewYork-Presbyterian Morgan Stanley Children's Hospital (CHONY ED) to provide asthma support to pediatric patients and their families. The Asthma Clinical and Educational Device, a mobile computer that utilizes a one-of-a-kind computerized curriculum to provide asthma assessment and customized asthma education directly to asthma patients and their families, has been available to patients in the CHONY ED starting in late November 2012. In addition, since mid-November a Community Health Worker offers on-site asthma support and can directly enroll and refer patients to the WIN for Asthma program. In February 2013, two WIN-BACK nurse practitioners also began providing clinical care, education, and support to pediatric asthma families and they can serve as a temporary medical home for these families, providing follow up asthma visits in the ED.

WIN-BACK has also established a multi-partner coalition which meets quarterly with community partners to address disparities and gaps in asthma care in Northern Manhattan. This Coalition leverages existing partnerships with community organizations, schools, and asthma programs and initiatives in northern Manhattan. With the help of this coalition WIN-BACK is expanding its efforts to provide improved pediatric asthma support and care at additional sites within the New York Presbyterian hospital network and the coalition's network of community partners.
