Ambulatory Care Network Community Health Education and Outreach

The Ambulatory Care Network's (ACN) Department of Community Health Education and Outreach helps safeguard and improve the health of many at-risk adults and children in the New York metropolitan area through programs such as the CHALK- Choosing Healthy & Active Lifestyles for Kids Program, the Community Health Nursing- Faith Based Partnership Program, the Family PEACE Program, the HEAL - Health Education & Adult Literacy Program, the Washington Heights/Inwood Network (WIN) Asthma Program, the Healthy Schools Healthy Families Program, the Turn 2 Us Program, the Lang Youth Medical Program, the Outreach Program, the Pharmacy Assistance Program (ACN-PAP), and the Reach Out and Read Program.

The Department of Community Health Education and Outreach also administers the Ambulatory Care Network Appointment Scheduling Center, a center that schedules appointments for patients and providers seeking care at New York-Presbyterian's Ambulatory Care Network's primary and specialty practices.
