Ambulatory Care Network's (ACN) Department of Community Health Education and Outreach helps safeguard and improve the health of many at-risk adults and children in the New York metropolitan area through programs such as:
- Choosing Healthy & Active Lifestyles for Kids Program (CHALK)
- Community Health Nursing - Faith Based Partnership Program
- Family Peace Program
- Health Education & Adult Literacy Program (HEAL)

- Lang Youth Medical Program

The ACN Department of Community Health Education and Outreach provides direct health care, mental health and health education services through the Center for Community Health and Education (CCHE) as it operates seven School-Based Health Centers, the community-based Family Planning Practice and the Young Men's Clinic.
The department also administers the ACN Appointment Scheduling Center. The center schedules appointments for patients and providers seeking care at NewYork-Presbyterian's ACN primary and specialty practices.
