Ambulatory Care Network Community Programs

Employing a community-centered model which includes longstanding partnerships with Columbia University Medical Center and Weill Cornell Medical College, the Ambulatory Care Network's (ACN) Division of Community Outreach mobilizes NYP's resources to provide clinical services, education, and preventative care to the members of the Hospital's communities in Northern Manhattan and the Upper East Side. Key to its mission is reaching individuals of all ages who are medically underserved and lack access to high quality health care.

The Division of Community Outreach is comprised of 15 programs providing services spanning from medical pipeline and youth education to HIV counseling and treatment. It is the home of the Center for Community Health and Education (CCHE) which administers seven (7) School-Based Centers serving young adults at 15 middle and high schools from the Bronx to Harlem. It also oversees the ACN Contact Center which fields over 40,000 calls a month and acts as a centralized point of contact for ACN patients. Together, these initiatives have a broad reach and help meet the Hospital's commitment to improving health care outcomes and increasing access for the members of its communities in a manner that underscores collaboration and community involvement.
