General Information
Applicants must have graduated from an accredited Doctor of Pharmacy (Pharm.D.) program and should be licensed or eligible for licensure in New York State prior to beginning the residency. All PGY-2 applicants must have a Doctor of Pharmacy degree and successfully completed a postgraduate year one (PGY-1) residency or equivalent experiences as approved by ASHP.
NewYork-Presbyterian Hospital Residency Programs participate in the ASHP-associated Pharmacy Online Residency Centralized Application Service (PhORCAS) for applications and the National Matching Service (NMS) for the residency matching process. Required documents should be submitted through PhORCAS by January 9, 2013. Required documents include:
- Letter of Intent
- Curriculum Vitae
- Transcripts
- Three Letters of Recommendation

Paper applications may be accepted under unusual circumstances through special arrangement with the Residency Program Director.
Letter of Recommendation Requirement
In addition to the standard reference material in PhORCAS, NewYork-Presbyterian Hospital Residency Programs require a letter of recommendation from each reference writer.
As an applicant, you will need to inform your references of this requirement.
Reference writers should address the following in their letter.
Completed letters must be submitted through PhORCAS by January 9, 2013.
