New York-Presbyterian Hospital (Hospital) and Columbia University Medical Center (CUMC) have learned of a security breach that may have resulted in exposure of health information, and in some cases, personal information of approximately 4900 of our patients. The Hospital and CUMC determined that a computer belonging to a CUMC researcher/Hospital lab clinician was stolen from a locked office at the hospital over the weekend of October 13-14, 2012. We have no evidence that this information has been accessed or misused.

The Hospital and CUMC have taken measures to reduce the likelihood of a future incident by further strengthening our information and data security systems and further enhancing our employees' awareness.

We deeply regret that any patient's information may have been exposed. We are notifying all of the patients affected by this possible exposure.

The confidentiality of our patients' information is of critical importance to us and we remain committed to the well-being and safety of our patients and to the security of all health information and personal information.
