Enroll in Benefits Here

You the Employee

As a full-time or part-time employee eligible for Hospital benefits, you may enroll in the NYP plans.

Your Spouse or Domestic Partner

Remember, if you and your spouse/domestic partner are both NYP benefit-eligible employees, the spouse/domestic partner earning the higher salary must enroll in medical coverage and elect Employee+1 or Family coverage level. The spouse/domestic partner must waive coverage.

Children

Benefits are available for your biological, adopted, or step-children, children for whom you are legal guardian, and any child(ren) you or your spouse/domestic partner is required to provide coverage for pursuant to a qualified medical child support order.

For Medical Insurance

Coverage is effective until the end of the year he/she becomes 26 years old.

Regardless of age, disabled unmarried children are eligible for medical coverage, if you submit the Empire Overage Dependent Form. Call Empire at (800-342-9816) to request the form.

For Dental & Child Life Insurance

Coverage is effective until the end of the year he/she becomes 19 years old, or until the end of the year he/she becomes 23 if enrolled as a full-time student. When you submit a claim for a dependent child age 20 through 23, the vendor will request documentation that he/she is a full-time student.
